In a large pot with a thick bottom (minimum 3 liters capacity), place the chopped apples, pears, and slices of ginger. Add the cinnamon sticks and cloves. If you are using optional ingredients, add the star anise and freshly grated orange zest now for extra aroma.
Pour all the ingredients into a pot with 1200 ml of filtered water. Place the pot on the stove over medium heat. Cover the pot with a lid and bring everything to a boil. When you notice that the water starts to bubble vigorously, reduce the heat to the minimum.
Cook the drink over very low heat for about 25 minutes. The liquid should only gently 'flicker' (slowly releasing small bubbles) and not boil vigorously. During this time, the fruits will completely soften, and the spices will release their deep aromas that will fill the entire kitchen. The drink will take on a beautiful amber color.
Remove the pot from the heat. Prepare a second container (e.g., a jug or bowl) and place a fine-mesh strainer over it. Very carefully pour the contents of the pot through the strainer to separate the clear liquid from the fruits and spices. You can gently press the fruits with a spoon against the strainer to extract as much juice and flavor as possible.
To the still hot, strained beverage, add honey or maple syrup if you're using it. Start with one tablespoon, mix thoroughly until completely dissolved, and taste. Add more if the drink is not sweet enough for you. Serve the elixir immediately, hot, in mugs or thick glass cups, garnished with a slice of fresh orange or an extra cinnamon stick.
